INTAKE INFORMATION

NEW INTAKES - The intake process is a clinical assessment that determines whether Medication-Assisted Treatment is right for you. A team of qualified clinicians and medical staff, including a counselor, nurse, and physician will gather health information and any related past and present drug use. Along with this assessment, patients will have their vital signs measured by a nurse and be evaluated by a physician. The physician will write your order based on your assessment and you will take your medication at the dosing window. Please be prepared to stay with us for 30 minutes after your first dose to ensure your safety.

 

 

INTAKES ARE BASED ON ORDER OF ARRIVAL

PLEASE ARRIVE BY 10:30AM TO GUARANTEE ADMISSION

 

 

TRANSFERS - Clients who transfer into the program will need to fill out a transfer/release form. Your transfer form will be faxed to your current clinic and your records must then be released by that clinic. Once medical records are obtained, your dose and phase will not change. Any take-home privileges or "PHASE" earned at a previous program will be verified and you will maintain that phase upon transfer.
